Vermont Immunization Requirements

The following immunizations are required by the state of Vermont for school attendance:

K - 6th

 

  • DTaP                5 doses

  • Polio                4 doses

  • Hepatitis B     3 doses

  • MMR                2 doses

  • Varicella         2 doses

7th -12th

          (in addition to what is required for K-6)

 

  • TdaP     1 dose
